Discharge Assessment

My patient is ready to be discharged, what do I need to do?


When your patient is appropriate for discharge, take the following steps:

  • If the patient has recently completed a reassessment that accurately reflects their current functional status and pain, use the “Mark as Discharge” action from the Assessments Section of the Participant’s Page. This action will label the appropriate assessment as “Discharge.”
  • If the the patient has NOT completed a recent assessment, send a reassessment. This action can be taken by selecting the action button next to a reassessment, and selecting “Send Reassessment.”
    • Once that assessment has been completed by the patient, use the “Mark as Discharge” action identified above.


Remember, patients continue to receive outcome measures for up to 2 years after their initial assessment.


If your patient elects to self-discharge, and would like to stop receiving assessments, take the following steps:

  • Mark the appropriate assessment as Discharge.
  • Use the “End Distribution” action. This action maintains the integrity and data associated with all previously completed assessments, and cancels all future assessments.
