I never received my Intake Assessment

  1. Verify the Patient’s Email and Phone Number in Limber.
  2. If correct, ask the patient to check their spam folder.
  3. Resend the Assessment or have the patient complete the assessment using the Scan QR Code pathway.


I entered the wrong information on my intake assessment

  1. Notify the Provider prior to taking the following steps
  2. On the Participant’s Assessment Section, select the action button next to the completed assessment. Choose Mark as Error. Provide the reason for the error.
  3. Manually Reassign the Outcomes Distribution from the Participant’s Assessment section.
  4. Click on the Action button at the top of the assessment section, and select “Send Additional Assessment.”
  5. From that list, select the Outcomes Distribution.


The link to my assessment expired

  1. Review the Participant’s Assessment section for “Missed” assessments.
  2. Resend the assessment by selecting the Action button, and choose to send reassessment.
  3. Note: For Reassessments, the Assessment will expire and be marked as missed after 7 days.


Why does my assessment say ‘Session Expired’?

  1. No, you can complete these assessments on both your computer, your phone, or any device which enables you to access the internet, like a tablet.
