QUESTIONNAIRE

A questionnaire is a collection of predefined questions based on domain standards or defined by Limber. These questions can be sourced from Global questionnaires or Original questionnaires. 


It's important to note that a questionnaire must always be part of a group (combination of a header and a questionnaire) in an assessment, and cannot exist independently.

ASSESSMENT 

An assessment consists of a collection of questionnaires that may include branching logic. It is made up of:

Multiple Groups

Multiple Headers (inside Groups or Questionnaires)

Multiple Questionnaires (inside Groups)


DISTRIBUTION

A distribution is the means by which assessments get delivered to participants. Distributions consist of one, or multiple assessments on specified cadences with specific communications and triggers. 

Ex: Outcomes Distribution (Outcomes Intake & Outcomes Reassessments, triggered automatically or manually)


OUTCOMES ONLY

Clients who are contracted to do Outcomes and NOT RTM are commonly referred to as “Outcomes Only”. In reality, Outcomes is sold alongside HEP for all and often does not mean the client is truly Outcomes Only.


PROVIDER PORTAL

This is the portal where providers will access patients and review completed outcomes.


OUTCOMES ADMIN

This is the Limber Internal admin portal where distributions, assessments and questionnaires are configured. 

This portal is managed by:

INTEGRATED

An integrated client, in regards to the Limber Outcomes system, is a client who has Patient and Case creation handled via SFTP or API integration directly from their EMR. This allows for the flow of IE dates into the Limber system for consumption by the Outcomes System.


NON-INTEGRATED

Non-integrated clients do not have patient and case creation informing fields in the Limber system. These groups are responsible for manually creating patients and their cases prior to the IE.


IE DATE (Initial Evaluation Date)

An IE is the date on which a patient is first seen by their rehab provider. IE’s are important to the Outcomes system as we look to gather two data points to calculate final outcomes:

  1. First outcome survey completed on or before initial evaluation date. This is the baseline measurement.

  2. Second outcome survey completed as close to the discharge date as possible. (See Discharge Date).

DISCHARGE DATE

A Discharge date is the date on which a patient stopped care. The goal is to have a patient complete an outcome survey as close to this date as possible so we can measure the true progression during care.


TRIGGER

Triggers are what tell the outcomes system WHEN to send an assessment to a patient. The most commonly used triggers in our system are:

  1. IE Date : Assessment is sent with a 48hr window prior to IE date

  2. Assignment of Program

CADENCE

Cadence describes the frequency at which new surveys are sent to patients. For example, the standard cadence is every 2 weeks for 12 weeks, 1x at 6m, 1x at 12m.


Integration vs. Non Integrated

The Outcomes system is designed to work in either integrated or non-integrated environments. The presence of integration decreases the lift on the staff as patients and cases will be automatically created in the Limber system. However, integration is not required for the use of the IE trigger
