Understanding the Initial Evaluation Check-In Workflow

Every new patient will receive outcomes prior to their initial evaluation. The system is set to automatically send email and text communications (if applicable) within 48 hours of the patient’s initial evaluation date. When the patient arrives for their visit, there are a few simple steps to take.


Check-In Workflow


Check the Participant’s Page on Limber

To see if a patient has an outstanding assessment, search for the participant, select the appropriate case, and review the Assessment section.


 


Review the Status of the Assessment Section on the Participant’s page


If they have completed the Intake Assessment, you will see the flag “Recently Completed”



If they have not completed the Intake Assessment, you will see the flag “Assessment Due” and the Outcomes Intake Assessment will be listed as Pending.

Provide the Assessment in Clinic

If the participant has not completed the Assessment, you have several options: Scan QR Code, Copy Link, Send Reminder (Email or Text).



Scan the QR code

After selecting the action to Scan QR code, a large QR code will appear on the computer screen. 


Open the camera function on the tablet, and ensure the code is within the line of sight of the camera.


A link will then become available for you to click and open the patient’s unique intake assessment.



Completion Page

When the participant has completed their assessment and reviewed their results, the following page will appear.


The flag on the Participant’s page should now change to “Recently Completed.”
