Answering patients’ common questions about Outcomes


What are outcome measures?


Outcome measures are surveys used to assess your current health status. We use them as a way to assess condition-specific considerations like mobility, function, and pain as well as general well- being, mental health, and quality of life.


Why are outcome measures important?


Outcome measures provide your Physical, or Occupational Therapist with valuable information regarding your health status, and help them determine the best course of treatment. Outcome measures are also one way your treating therapist will track your progress over time.


How will I receive outcome measures?


A unique link for each assessment will be sent to you via email (support@limberhealth.com) and/or text message. Simply click on the link to complete the questions. If you are unable to complete the assessments at home, you will be asked to complete them during your visit.


How long will the outcome measures take to complete?


The very first assessment will take ~3 minutes to complete. All follow-up assessments will take ~1 minute.


How often will I be asked to complete outcome measures?


You will be asked to complete the first assessment two days prior to your initial appointment. Every two weeks during your care, you will be sent a brief assessment to track your progress. After you have completed your in-clinic care, you will periodically continue to receive these assessments for up to two years to ensure you continue to progress.


Can I opt out of completing outcome measures?


You are able to “unsubscribe” from both the assessment email and text message. Please keep in mind, however, that outcome measures provide important information to your treating therapist to give you the best care possible.
